Consent
What you are agreeing to
Three separate things, kept separate on purpose. Two are necessary to treat you safely. The third is entirely optional and never affects your treatment.
Last updated 9 October 2026
1 · Consent to treatment
- What it covers
- Agreement to the specific treatment planned for the day, after it has been explained to you — what it does, how it will feel, what to expect afterwards, and what the alternatives are.
- When it happens
- At the appointment, in writing, before anything begins. If the plan changes partway — a different modality, an added extraction — you are asked again.
- You can stop
- At any point, for any reason, without explaining yourself. Say so and we stop.
- Under 18
- A parent or guardian must be present and must consent.
2 · Health disclosure
- What we ask
- Medications, allergies, recent procedures, skin conditions, pregnancy, and anything else that changes what is safe to do. This is the contraindication screening.
- Why it is not optional
- Several things on the menu are genuinely unsafe in combination with common medications and recent treatments. This is the one piece of information that can prevent harm, and it only works if it is complete.
- Nothing is judged
- It changes what is recommended. That is all it does.
- It may change the plan
- A treatment can be adjusted or declined on the day. Your deposit carries to a suitable alternative — you are never penalised for being honest.
3 · Photographs — optional
- Never a condition
- You can decline and nothing about your treatment changes. You are not asked twice in the same appointment.
- Separate permissions
- Progress photos kept in your own record, and photographs used publicly, are two different permissions. Agreeing to the first does not give the second.
- Written, and recorded
- No image is taken or used without written consent recorded at the appointment.
- Never altered
- Before-and-after images are never retouched, filtered or smoothed. A result that needs editing is not a result.
- Withdraw at any time
- Email purposeskinmotion@gmail.com and we remove the images from anywhere we control. Anything already reshared by someone else is outside our reach, which is worth knowing before you agree.
Where it is kept
- With your record
- Consent forms are kept with your treatment record, for as long as Georgia law and our insurer require. See privacy.
- Ask for a copy
- Any time. It is yours.
